About the USC Geriatrics Healthcare Collective Program (GHC)
The growing older adult population coupled with a shortage of an adequately trained health care workforce requires innovative models of care delivery and training to create a geriatric competent primary care workforce. The Geriatrics Healthcare Collective Program (GHC) is a five year, $3.5 million grant from the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS). USC GHC is developing comprehensive interprofessional geriatric primary care-based assessment and training programs with an emphasis on interprofessional training and team-based care. Our focus is on improving recognition and care for patients who have cognitive impairment or have a dementia diagnosis, and who, in addition, have significant medical co-morbidities.
In addition to Alzheimer’s Greater Los Angeles, LGBT Center, Eisner Health, and Wise & Healthy Aging, and our community housing partners, TELACU and Menorah House, the Keck School of Medicine is also collaborating with the USC School of Social Work, USC School of Pharmacy, Mrs. T.H. Chan Division of Occupational Science & Occupational Therapy, and USC Division of Biokinesiology and Physical Therapy.
